Executive Order No. 38/2024 Establishment of UniCare and Project MediPrivate (Yakcare)
By the authority vested in me as President of the Commonwealth of Redmont, I hereby issue the following Executive Order:Section 1: Establishment of UniCare
(a) Purpose:UniCare is a comprehensive healthcare framework designed to ensure equitable access to healthcare for all citizens while fostering a sustainable partnership between public and private healthcare providers. The program will address gaps in public healthcare availability by supporting private providers financially, improving overall healthcare quality, and ensuring affordability for citizens.
(b) Public Healthcare (Medicare):
- Medicare covers treatments provided at government-operated hospitals and clinics.
- Citizens may seek treatment at private healthcare facilities. The Government will provide financial support through the Medicare Co-Payment System.
- Private doctors and facilities must comply with DOH standards and regulations to participate in this system.
Section 2: Medicare Co-Payment System
(a) Definition:The Medicare Co-Payment System allows the Government to subsidize treatment costs at private healthcare facilities based on a predefined Medicare Schedule.
(b) Payment Types:
- Bulk Billing: The Government fully covers the treatment cost under Medicare.
- Example: A doctor charges $100, according to the Medicare Benefits schedule for that treatment, and the Government pays the full $100 directly.
- Mixed Payment: The Government covers 50% of the total treatment cost (the threshold). The remaining balance is paid out-of-pocket by the citizen.
- Example: A private firm may quote $500 to be treated, the treatment under the Medicare Co-Payment schedule states that treatment attracts a co-payment of $50.
- That means the Doctor can charge $50 from the player in co-payment.
- The remaining $450 has to be paid by the player.
- Example: A private firm may quote $500 to be treated, the treatment under the Medicare Co-Payment schedule states that treatment attracts a co-payment of $50.
- Fully Private: The Government provides no coverage. Citizens must pay the full cost out-of-pocket.
- The DOH, will publish and regularly update a Medicare Benefits Schedule, which will include:
- A list of treatments and procedures eligible for Medicare Co-Payment.
- The amount per treatment and procedures covered under Medicare Co-Payment.
- Clear definitions for treatments to prevent abuse or misclassification.
- Cost Transparency: Private doctors must state the treatment cost upfront and clarify whether it qualifies for:
- Bulk Billing,
- Mixed Payment, or
- Fully Private.
- Patient Rights: Citizens may refuse treatment before it begins without penalty.
- Payment Collection: The doctor may then execute a command to claim the Government co-payment.
Section 3: Standards of Care and Regulation
(a) DOH Oversight:- The DOH shall regulate and monitor all healthcare providers participating in the UniCare system.
- Cost Transparency: Doctors must provide a clear, upfront cost for treatment and specify Medicare coverage details.
- Treatment Approval: Doctors must receive verbal consent from the patient before proceeding with treatment.
- Co-Payment Fraud Prevention:
- Doctors cannot claim co-payment for treatments that do not qualify under Medicare.
- Pricing must include co-payment; additional hidden fees are strictly prohibited.
- Citizens have the right to:
- Know the cost and Medicare coverage before treatment begins.
- Refuse treatment at any point before the procedure starts.
- Report providers who fail to comply with DOH standards.
- The DOH shall audit providers for compliance with regulations.
- Providers found engaging in fraudulent activity will:
- Have their licenses revoked.
- Be referred to the DOJ for further investigation and prosecution.
Section 4: Doctor Qualification and Licensing
(a) Government and Private Exams:- Government Doctor Exam: The existing exam for Government doctors will remain unchanged.
- Private Doctor Qualification Exam: A new exam will test knowledge of:
- Medical treatments and guidelines,
- Medicare Co-Payment procedures,
- DOH standards of care, including cost transparency and patient rights.
- Doctors must renew their licenses every 3 months.
- The DOH may revoke licenses if:
- A doctor has been inactive for 30 days or more.
- The doctor fails to meet DOH standards or engages in misconduct.
- The DOH may use a command to remove licenses for non-compliance.
- Revoked doctors may reapply after a mandatory review period of 7 days.
Section 5: Implementation and Compliance
(a) System Development:- The DOH will implement:
- A command for private doctors to claim co-payment.
- A command-based system for license enforcement.
- The DOH will conduct monthly audits of private healthcare providers to ensure compliance.
- Citizens may report non-compliant doctors directly to the DOH through a department ticket.
Section 6: Resource Allocation and Funding
(a) Budget Allocation:- This executive order establishes the Redmont Healthcare Fund (RHF) to fund Medicare Co-Payments for private healthcare providers.
- This fund will be an allocation under the DOH’s appropriated funds, exclusively for co-payment.
- Private providers may source medical supplies independently but must adhere to DOH-approved quality standards.
Section 7: Enactment
This Executive Order supersedes all previous healthcare-related executive orders and shall take effect immediately.One Vision, One Purpose, One Dream.
